The book (a handsome hardcover) has in it the 5 tips I've given you plus
much, much more. Stories, anecdotes, interviews, check lists, charts.
A tongue-in-cheek glossary, bonus resources that include corporate blogging
guidelines and pointers to legal resources and even an illustrated
anatomy of a blog.
And one of my favorite parts, an excellent index. Have no idea
how the publisher creates this, but this one is a perfect guide
to the book. Look up "CEO bloggers," for example, and you'll
find 19 references to related topics: ghostwriters, objections to,
statistics on, etc.

after counseling thousands of managers and execs on how

Just start a blog. Get someone to help you set up a TypePad
account (www.typepad.com) although it's fairly easy to do yourself.
There are a number of blogging platforms to choose from
but I continue to recommend TypePad. It has all the bells and
whistles built into it. And it's hosted on SixApart's servers so it's
a no fuss - no mess way to get started.
So get the techology part taken care of (this is the easiest part,
I promise). And just start blogging. I.e. posting short entries to
your blog and experimenting with writing style and topic.
